Consent for Treatment Form 4. Financial Policy MOTOR VEHICLE ACCIDENT PATIENTS: If you are being seen for a motor vehicle accident, please fill out all of the above forms. Additionally, please complete the Motor Vehicle Accident Form and the Personal Injury Form.
